1. Acceptance of these Terms
These Website Terms of Use constitute a legally binding agreement between you and Raven's Eye Mental Wellness and Nursing Services, Inc., doing business as Raven's Eye Wellness, together with its owners, officers, clinicians, employees, contractors, representatives, successors, and permitted assigns, collectively referred to in these Terms as “Raven’s Eye,” “we,” “us,” or “our.”
These Terms govern your access to and use of this website, its webpages, educational materials, public contact forms, scheduling links, online resources, and other digital services that expressly link to these Terms, collectively referred to as the “Site.”
By accessing, browsing, submitting information through, or otherwise using the Site, you acknowledge that you have read, understood, and agreed to be bound by these Terms and the separately posted Legal & Privacy Notice. If you do not agree to these Terms, you must discontinue use of the Site.
2. Relationship to Patient and Clinical Agreements
These Terms govern the public Site. They do not replace or modify any Notice of Privacy Practices, informed-consent document, telehealth consent, patient services agreement, financial policy, cancellation policy, medication policy, controlled-substance policy, treatment agreement, authorization, or other clinical document provided separately by Raven’s Eye.
When a person becomes a patient, the patient relationship will also be governed by the clinical and administrative documents presented during intake, through the patient portal, or directly by a Raven’s Eye clinician or authorized representative.
If these Terms conflict with a legally required Notice of Privacy Practices, signed patient agreement, or nonwaivable requirement of applicable healthcare law, the applicable legal requirement or patient-specific agreement will control with respect to the subject of that conflict.
3. Informational Purposes Only
The Site provides general educational, administrative, and informational material concerning Raven’s Eye and the types of services that may be available.
The Site and its contents are not and shall not be interpreted as:
- Medical, psychiatric, psychological, nursing, or pharmaceutical advice;
- An individualized diagnosis or treatment recommendation;
- A direction to begin, discontinue, increase, decrease, or otherwise change medication;
- A substitute for an evaluation by an appropriately licensed healthcare professional;
- A guarantee that a particular treatment, medication, provider, or service is appropriate for any individual;
- A guarantee of insurance coverage, reimbursement, appointment availability, or acceptance as a patient; or
- A guarantee of any particular clinical, functional, financial, or treatment outcome.
You should not disregard professional healthcare advice or delay seeking appropriate care because of information appearing on the Site.
4. No Provider-Patient Relationship Through Website Use
Viewing the Site, submitting a contact form, sending an email or text message, leaving a voicemail, requesting an appointment, or interacting with a third-party scheduling or directory platform does not, by itself, establish a provider-patient relationship.
A provider-patient relationship is established only after Raven’s Eye has completed the steps reasonably required under the circumstances, which may include:
- Confirming that an appropriate clinician is available;
- Confirming that the clinician is legally authorized to provide services in the patient’s physical location;
- Determining that the requested service is clinically appropriate;
- Verifying identity, contact information, location, insurance, or eligibility information;
- Completing required intake, consent, privacy, and financial documentation;
- Accepting the individual as a patient; and
- Commencing an appropriate clinical evaluation or encounter.
Raven’s Eye may decline an inquiry or request for services when permitted by law, including because of clinician availability, licensing restrictions, clinical appropriateness, inability to verify required information, insurance limitations, geographic location, or the need for a different or higher level of care.
5. Emergencies and Urgent Communications
THE SITE, PUBLIC CONTACT FORMS, ORDINARY EMAIL, SMS TEXT MESSAGES, VOICEMAIL, SCHEDULING SYSTEMS, AND ROUTINE PATIENT-PORTAL MESSAGES ARE NOT EMERGENCY SERVICES AND MAY NOT BE MONITORED CONTINUOUSLY.
Do not use the Site or routine electronic communications to report an emergency, an immediate threat, active suicidal intent, active homicidal intent, a suspected overdose, a severe medication reaction, rapidly worsening symptoms, or another condition requiring immediate intervention.
If you are experiencing an immediate emergency, call 911 or go to the nearest emergency department. In the United States, individuals experiencing a suicide, mental-health, or substance-use crisis may call or text 988.
The availability of emergency-resource information on the Site does not make Raven’s Eye responsible for providing emergency dispatch, crisis response, or continuous monitoring services.
6. Telehealth Services and Patient Location
Raven’s Eye may offer telehealth services to eligible patients located in California and Washington when the treating clinician is properly licensed or otherwise legally authorized to provide the requested service in the state where the patient is physically located at the time of the encounter.
Telehealth services may be provided only when:
- The patient’s identity and physical location can be reasonably verified;
- The treating clinician is legally authorized to practice in that location;
- Telehealth is clinically appropriate for the patient and requested service;
- Required intake, consent, privacy, and financial documents have been completed; and
- The patient has access to technology and an environment reasonably suitable for the encounter.
The fact that the Site can be accessed from another state, territory, or country does not represent that Raven’s Eye offers or is legally authorized to offer clinical services in that jurisdiction.
7. Appointments, Cancellations, Insurance, and Payment
Appointment, cancellation, late-cancellation, no-show, insurance, billing, payment, refund, and collection policies are provided separately during intake, through the patient portal, or through Raven’s Eye’s Insurance & Payment information.
Fees, insurance participation, benefits, deductibles, copayments, coinsurance, Good Faith Estimates, and other financial information may depend on the patient’s circumstances and are not guaranteed solely because information appears on the Site.
Patients remain responsible for reviewing their financial agreements and for providing accurate and current insurance and billing information. A statement that Raven’s Eye accepts or works with an insurance organization does not guarantee payment by that organization for a particular service.
8. Third-Party Platforms and Services
Raven’s Eye may use or link to independent third-party services for provider directories, referrals, appointment requests, scheduling, intake, identity verification, telehealth, patient communications, insurance administration, billing, payment processing, electronic records, transcription, or document management.
Depending on the services selected by Raven’s Eye or the patient, these platforms may include IntakeQ, Headway, Zocdoc, Psychology Today, payment processors, telecommunications providers, email providers, hosting providers, analytics providers, or other contractors.
Each third-party platform may maintain its own terms, privacy notice, cookie practices, account requirements, security procedures, and technical limitations. Information collected directly by a third-party platform may be governed by that platform’s policies before Raven’s Eye receives it.
Raven’s Eye is not responsible for every independent act, statement, availability decision, advertisement, security practice, or policy of a third-party platform. A link or reference to a third party does not constitute an endorsement or guarantee of every product, statement, or business practice of that third party.
9. Electronic Communications
By voluntarily providing an email address or telephone number, you authorize Raven’s Eye to use that information for reasonably related administrative communications, subject to applicable law and your communication preferences.
These communications may include:
- Responses to inquiries;
- Appointment requests, confirmations, reminders, or changes;
- Requests to complete forms or provide documents;
- Patient-portal notifications;
- Insurance or billing communications;
- Care-coordination communications permitted by law; and
- Security, privacy, operational, or service notices.
Ordinary email and SMS text messages may present privacy and security risks. Messages may be delayed, misdirected, intercepted, displayed on a shared device, or accessed by a telecommunications, email, mobile-device, or operating-system provider.
Consent to receive ordinary administrative communications does not constitute consent to receive unlawful marketing communications and does not, by itself, constitute consent to record or AI-transcribe a confidential communication.
10. Artificial Intelligence, Automation, and Analytics
Raven’s Eye may use appropriately configured artificial intelligence, machine-learning, language-processing, automation, transcription, and analytics tools to support administrative, operational, security, and clinical workflows.
Potential uses may include:
- Routing, categorizing, or prioritizing inquiries;
- Filtering spam, malware, phishing, or suspected fraud;
- Assisting with scheduling and intake administration;
- Summarizing patient-provided or administrative information;
- Preparing draft administrative communications;
- Assisting an authorized clinician with draft documentation;
- Creating a draft transcript or draft note following appropriate notice and consent;
- Measuring workflow, response time, call routing, or service performance;
- Analyzing website traffic, reliability, security, and technical performance; and
- Identifying errors, outages, abuse, or cybersecurity events.
AI-generated or automated output may be incomplete, inaccurate, mistranscribed, misclassified, or otherwise inappropriate. AI output is not a substitute for professional judgment.
Raven’s Eye does not authorize an AI system to autonomously diagnose a patient, autonomously prescribe medication, or make an unreviewed final clinical decision. An appropriately authorized human professional remains responsible for material clinical decisions and for reviewing AI-assisted clinical content before relying upon it.

11. Recording and Transcription of Calls or Meetings
The publication of these Terms does not, by itself, constitute consent to record a confidential telephone call, video meeting, telehealth encounter, or other confidential communication.
Before Raven’s Eye records, live-transcribes, or transmits the substantive contents of a confidential communication to an AI-assisted transcription or documentation system, Raven’s Eye will provide an additional notice and obtain consent when required by applicable law or Raven’s Eye policy.
The additional notice may identify:
- That recording, transcription, or AI-assisted processing will occur;
- The general purpose of the processing;
- Whether audio or video will be retained;
- Whether an approved technology provider will process the information;
- That automated output may contain errors; and
- How the participant may ask questions, decline, or withdraw consent prospectively.
Where recording or AI transcription is optional, declining it will not, by itself, cause Raven’s Eye to deny otherwise available care. Raven’s Eye may instead use manual documentation or another lawful method.
Raven’s Eye may collect limited technical call information such as call date, time, duration, routing, connection quality, or abandonment information for administrative, operational, and security purposes without recording the substantive contents of the call, where permitted by law.
12. Public Forms and Sensitive Information
Unless a form is expressly identified as a secure patient-intake or patient-portal function, do not submit detailed medical records, psychotherapy notes, Social Security numbers, complete payment-card information, government identification documents, account passwords, or other highly sensitive information through a public Site form.
Submitting information through a public form does not guarantee that the information will be reviewed immediately or by a clinician. Raven’s Eye may move the communication to a secure portal, telephone call, or another approved communication method before discussing sensitive information.
13. Permitted and Prohibited Use of the Site
Raven’s Eye grants you a limited, revocable, nonexclusive, nontransferable license to access and use the Site for lawful, personal, noncommercial, and informational purposes.
You may not:
- Use the Site in violation of any applicable law or regulation;
- Impersonate another person or misrepresent your identity or authority;
- Submit materially false, fraudulent, threatening, abusive, or misleading information;
- Attempt to access another person’s account, records, or communications;
- Attempt to bypass authentication, security, rate limits, or access controls;
- Introduce malware, destructive code, or unauthorized automated requests;
- Scrape, crawl, index, copy, or extract Site content for unauthorized commercial use;
- Interfere with Site operation, availability, hosting, or security;
- Use the Site to harass, threaten, defraud, or unlawfully discriminate; or
- Use Site content to provide misleading medical or legal representations.
Raven’s Eye may restrict or block access when it reasonably believes the Site is being misused, attacked, accessed unlawfully, or used in a manner that threatens patients, staff, systems, or third parties.
14. Intellectual Property
Unless otherwise stated, the Site and its original content, including text, branding, logos, graphics, photographs, videos, layouts, software components, design elements, and educational materials, are owned by or licensed to Raven's Eye Mental Wellness and Nursing Services, Inc. and are protected by applicable intellectual-property laws.
You may not reproduce, republish, distribute, sell, modify, frame, mirror, publicly display, create derivative works from, or commercially exploit protected Site content without prior written authorization, except as expressly permitted by applicable law.
Third-party names, trademarks, service marks, logos, and content remain the property of their respective owners.
15. Website Availability and Changes
Raven’s Eye may modify, suspend, restrict, or discontinue any Site feature or content without prior notice. Raven’s Eye does not warrant that the Site will be continuously available, error-free, compatible with every device, or free from every harmful component.
Website information may become outdated because of changes in law, licensing, insurance participation, provider availability, technology, clinical practice, or third-party services. Raven’s Eye does not undertake a duty to update every webpage immediately after such a change.
16. Disclaimer of Website Warranties
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THE PUBLIC SITE IS PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS.
Raven’s Eye disclaims express, implied, and statutory warranties relating solely to the public Site, including implied warranties of merchantability, fitness for a particular purpose, title, noninfringement, availability, accuracy, and compatibility.
Nothing in this section disclaims a professional, privacy, security, or healthcare duty that cannot lawfully be waived, including duties arising from an established provider-patient relationship.
17. Limitation of Website Liability
To the maximum extent permitted by applicable law, Raven’s Eye shall not be liable for indirect, incidental, special, exemplary, punitive, or consequential damages arising solely from access to, use of, or inability to use the public Site.
This limitation may include loss arising solely from Site interruption, outdated general information, independent third-party links, unauthorized access caused by a user’s failure to protect credentials, or reliance on general educational content rather than individualized professional advice.
This limitation does not apply to liability that cannot legally be excluded. It does not alter a patient’s rights concerning professional healthcare services, medical negligence, privacy, security, breach notification, discrimination, professional misconduct, or other nonwaivable legal duties.
18. Responsibility for Intentional Misuse
You are responsible for losses, claims, liabilities, or reasonable expenses directly resulting from your intentional unlawful misuse of the Site, your intentional violation of another person’s rights, or your intentional introduction of malicious software or unauthorized access activity.
This section does not require a patient or consumer to indemnify Raven’s Eye for Raven’s Eye’s own negligence, professional conduct, unlawful privacy practice, security failure, or violation of a statutory duty that cannot legally be waived.
19. Governing Law and Nonwaivable Rights
Raven’s Eye is a California entity that may provide eligible services to patients located in California and Washington.
These Terms shall be interpreted under applicable federal law and, to the extent permitted, the laws of the State of California, without depriving a Washington resident or another consumer of mandatory protections that cannot lawfully be waived under the law applicable to that person.
Nothing in these Terms limits a person’s right to submit a complaint to a healthcare licensing board, privacy regulator, attorney general, health plan, law-enforcement authority, accrediting organization, or other government agency.
20. Changes to These Terms
Raven’s Eye may revise these Terms to reflect changes in law, technology, vendors, services, business practices, or Site functionality.
The current version will be posted on the Site with an updated effective date. Where required by law, material changes affecting existing patients may also be communicated through the patient portal, email, written notice, or another appropriate method.
Continued use of the public Site after revised Terms are posted constitutes acceptance of the revised Terms, except where applicable law requires a different form of notice or consent.
21. Severability and No Waiver
If any provision of these Terms is determined to be invalid, unlawful, or unenforceable, that provision will be limited or reformed only to the minimum extent necessary, and the remaining provisions will remain in effect.
A failure by Raven’s Eye to enforce a provision on one occasion does not constitute a waiver of that provision or of any other right.

Do not send emergency or time-sensitive clinical communications to the legal contact address. Emergency and urgent communications must be directed to the appropriate emergency or clinical resources.
